Hello,
A quick question on calculating the Risk Score. Correct me if any of this is wrong, since that could be part of my confusion on the subject. My understanding is that patients with ESRD use HCC V21. The current HCC version is V24 for all other Medicare patients. Would ESRD patients ever be assessed with both HCC V24 and V21? If they are, are the two risk scores from both versions added together or would they be separate scores? Is HCC Rx Risk Score kept as a separate score or is this added with either of the other two versions?

ESRD is calculated at 136 for CMS 21, 22 and 24. So no, you would not add them together. The previous poster is correct, you will have to follow your organization's rules.
