Here is the question I am coding HCC records and I want to know if a dx is not in the assessment area of the record can we or should we code it anyway because its on the progress note? My co-workers and I have been seeing alot of this in some of the records we have.

If it is a chronic condition then yes, but also look for supporting information to help support this DX such as lab work or med. prescribed. But also, if the code is in the HCC list as well. Good Luck!!
