Wiki HD effecting antibiotics

I have an ID doc that is being asked to consult for an antibiotic verification. The pt is on 8-12 hrs per day HD and that is basically depleting the antibiotics out of the pts system. I am thinking along the 999.9 side; does anyone else have any suggestions?

It's not really a therapeutic poisoning. It is more like they are monitoring to prevent it because the patient's HD treatments are not allowing the anitbiotics to work to their highest potential. The HD is washing out the antibiotics before they are able to work.
