Healthfirst denying cpt code 58300 (iud insert) Missing NDC number, we did not supply iud only inserted.
In addition 58300 does not require NDC number.

Anyone having similar issue?
 
Many insurances will require the Mirena J code along with 58300 (similar to injection codes). If you did not supply the IUD, bill the J7298 with 0 fee. If your software won't submit a code with 0 fee, then make it .01. The NDC number (likely 50419-423-01 but check your packaging) goes on the J7298.

Only insurance we're having an issue with is Healthfirst.

If we do NOT supply IUD and just insert insurance companies will pay for the insert only without any issues.

This is the first and only insurance that we have issues with
