Recently, Healthfirst has changed their policy for CPT 96910 and now it needs preauthorization.
But we are getting so many denials and the reason is not clear.
Please attached portion of the denial letter.
Nothing is clear and agent couldn't explain either.
Does anyone familiar with MCG Guideline?
I would need help if anyone has more information.
