Wiki Hearing Exam Codes

Please help with appropriate use of V5008, 92551 and 92552.

Do any or all of these codes need to be billed using a hearing booth?

I have read the CPT descriptions and they are not very clear on what documentation is required for each code.

If anyone out there has experience/expertise in an audiology practice any input/advice is greatly appreciated.
