Can you help clarify if 93454 vs. 93458 is more appropriate in this scenario? Provider indicates he has taken the LVEDP pressures and records them in the findings; however he does not document the catheter went across the valve for an LV shot or even that the catheter crossed the valve (to support a true Left Heart procedure). I have search CPT assistant and am unable to locate a reference for this particular scenario - if you have an official reference it would also be appreciated. Thank you!
 
The only way to get a LVEDP is by putting a catheter in the LV and taking pressures. However, you should tell the physician to document the catheter positions so the procedure can be coded properly.
