HEDIS Weight Assessment and Counseling for Nutrition

Does anyone know if, in order to meet HEDIS guidelines, a well care visit (for ages 3-17 with a BMI percentile documentation) has to have a dietitian give counseling (CPT 97802-97804?)

Or can the provider just document that he/she discussed nutrition with the patient, and then enter Z71.3 as one of the diagnoses, and use CPT 99381-99397?


Any help would be appreciated. Thanks
