Does anyone have suggestions on books or resources for E/M coding other than the Medicare guidelines? I'm looking for something with details. I am working with physicians that have never had any instruction on how to document properly for coding and they like to question things especially the medical necessity of what they are doing and the MDM.

There is a website called E/M University that has great info.
I purchased the auditing manual from NAMAS on Amazon for $89.00 that explains E/M coding/documentation in detail.
 
CPT 76000 Question

Hello,

I received a denial because CPT code 76000 was coded separately from CPT code 74000. I've been asked to advise on the modifier. From my understanding, code 76000 is bundled and should not be coded separately. Should I reverse the charge and leave 74000 as is or should I append a modifier to the 76000 to indicate that it's bundled with 74000? Thanks!
