I am a little confused on how to code a routine screening pap in our office for a commercial insurance patient. No Preventative visit was done. She just needed a pap.

Collection for a screening pap smear is normally included in E/M reimbursement. Q0091 may be billed to both Medicare and private insurances, but it is rare to be paid by commercial insurances. Of course, the pap smear itself must be billed by the laboratory.
