I have a few Eye Drops that I have been unable to fine HCPCS codes for so I can code for them. I'm Wondering if any of you have the associating J codes for them.
They are....
Fluorescein Sodium and Benoxiate Hydrochloride Ophtalmic Solution USP 0.25%/0.4% NDC 24208-*732-05

Gentamicin Sulfate Ophthalmic Solution USP 0.3% NDC 24208-*580-60

Proparacaine Hydrochloride Ophthalmic Solution USP 0.5% NDC 24208-*730-06
