My boss is telling me that it is ok to code from a superbill. The patient had a Podiatry consult and the note has no diagnoses in it. However, the provider hand wrote E11.51 on the Superbill. She's saying that is enough to report the code. I know this is wrong but I need documentation to show her. Please help!
 
The note doesn't need the diagnosis code written in it, but it does need to address a diagnosis (as in, it doesn't need "E11.51" written but it should have "DM2 with PAD" written in there).

ETA: You can't code from a superbill without a note, but you can code from a note without a superbill
