Having difficulties getting medicare and blue cross to pay for aloxi using cancer codes and chemo encounter. Any info would be great.

Your Aloxi must be coded 787.01/DX/v58.11, providing, I am assuming it is given as an anti- emetic along with the chemo infusion? You would also code it as a 90767(sequential) or 90768(concurrent), unless 90765 applies.
