What is the appropriate bill type (UB04 box 4) for revenue code 1002 and HCPCS code H0018?

Try using bill type 111, unless is BCBS. If your sending claims to BCBS u need to use 112 for 1st inpatient day, 113 for every day after, and 114 for the final claim.
 
Billing

When I bill with the revenue code 1002 on the UB-04 claim I leave out the H0018 and it gets paid (BCBS and Bridgespan do not like the H codes). This is only for commercial insurance though since my facility does not bill for Medicare or Medicaid.
