Without seeing a copy of the op report, my recommendation would be to take a look at CPTs 64831 & 64834.

64831 is digital nerve, 64834 is common sensory nerve.

If using 64831, the add-on for the 2nd nerve is 64832. If using 64834, the add-on for the 2nd nerve is 64837.
