Help in coding L8680

Hi,

I come across a scenario where I should be coding L8680 but the op note states as below so I am not sure how many units i should be coded is it 2 or 16?

T12/L1 Entry - Lead Placement T7-T9 x2 16 Contact.

This is for a Med advantage claim and also let me know whether there is any changes to billing commercial policies
