Help! Medicare and Podiatry. New coder in this!

Hi All,
I am new to the podiatry world and am having MAJOR issues getting paid from Medicare (i am in NY). For 11721 and 11056 they want the last time the patient saw the PCP. Now most of the patients will say "about a month ago" or they dont know. Can anyone give me any input on how your office gets this info? I just dont know the best way to do this! Thanks!!

You have to call the doctor's office and get the information. Pam

You have to have seen your PCP in the past 6 months. Try to educate what ever staff rooms the patient to get this information from the patient. We always tell them we need this info or the visit for today will not be paid. You will be surprise how fast they remember when the think they will have to pay.

How/where/what field do you use to indicate the last time the PCP was seen?

There is a field in my EMR system that says- last seen date. All systems are different. It appears in box 15 of the HICFA.
