For the following procedure:
Percutaneous tenotomy of the hip gluteal (abductor and extensor) tendon with ultrasound guidance for device localization.

What is the appropriate CPT code?

27000 is for (adductor)
27006 states "open"

Should I use an unlisted code?

I would recommend to use unlisted for per-cutaneous tenotomy gluteal muscle.
For open we have code 27006 but for per-cuteaneous we don't have any CPT code, hence recommend to use unlisted.
As gluteal muscle is divided into two group :
1. Superficial abductors and extensors
2. Deep lateral rotators
