Hello, I need some help with the following dx coding of a nerve conduction study:

1) There is electrical evidence of a motor polyradiculopathy of the right arm, with segmental denervation, more distally than proximally. It affects muscles sharing C7, C8, T1 innervation. A motor neuron disorder cannot be excluded.
2) There is electrical evidence of a mild right ulnar neuropathy, but out of proportion to the degree of denervation of the first dorsal interosseous, which is severe.

Any help is much appreciated!

"electrical evidence of..." doesn't necessarily mean that the physician has made a definitive diagnosis. Probably best to check with the physician. If no definitive diagnosis can be made, then you need to default back to the ICD-10 code(s) for the patient's signs & symptoms that was the basis for performing the diagnostic test.
