Yes, you're correct, you might wanna add modifier 59 on your LHC if it's really diagnostic. Also, if the stent is DES, you might have to code it on C9600-C9608.

Diagonal is a branch of the LAD. So the numbers should be reversed. LHC - 93458-XU, 92928-LD or C9600-LD for DES, and 92929-LD for diagonal branch or C9601-LD for drug eluting stent. (DES charges are for hospital, 92xxx is for physician)
