I am debating on what code to use for two diffrent things.

1. A dipstick urinalysis for ketones. Should I use 81000 or 81002 and what is the diffrence?

2. Fecal occult blood, three determinations by guaiac. Should I use 82270 or 82272. And what is the diffrence beetween the two?

If the physician viewed the urine under a microscope it would be 81000 if it was just a dipstick it would be 81002.

Addendum:
Medicare rarely pays for a 81002 because it is no longer a CLIA waived test. Most UAs need to have a microscope so they will be paid.
