My questions to you are, is this patient in a SNF? (Nursing Home) ?
An assisted living facility? Or Home, or Private Residence of patient?

Additionally, the CPT code 94760 is included with CPT 99344. Also, CPT code 99407is included with CPT code 99344. Although you are reporting modifier 25, make sure the documentation clearly identifies a significantly separate and identifiable service aside from the home visit.

Also, last question, what is the denial code that Medicare provided to you?

I am also trying to find out if we can bill the 99406/99407 on a patient we see in the nursing home or skilled nursing facility.
