Just trying to see if I am correct? It seems my providers (cardiologists & cardiothoracic surgeons) mostly perform "comprehensive" HPI's and PFSH in the history section of the E/M, however their ROS usually falls into "detailed". Am I correct that scoring of this section would be "detailed" since - "No column contains three circles, the column containg a circle farthest to the left, identifies the type of history"???

Yes, you are correct - all three parts of the History key element must meet the comprehensive level for the history to be comprehensive. Since the ROS is detailed and the other two are comprehensive, the History key element will be detailed.
