Hi,
Can anyone help? I have a claim where a 26 modifier was added to the procedure code 95941, and was denied as invalid modifier. I cannot figure out why this would have been added in the first place. Is there a justifiable reason for adding this modifier to this code?

According to the 2013 RVU Files, 95941 (and also the similar 95940) does not have a TC/26 breakdown, so you should be able to bill it appropriately without the 26 modifier. I can't think of any reason it should be added, unless whoever did it assumed it needed the 26 since the code it replaced also did.
