Many organizations have a policy of billing in RVU order (greatest to smallest). I'm not even sure if it's necessary to do that anymore now that claims processing technology has gotten more sophisticated, but I still bill that way out of habit. I first started billing in the days when you had to rank them in order to get paid correctly.

Remember also that there's an NCCI edit between 43254 and 43251. You'll need to review your documentation to determine whether a modifier is appropriate.
