Excision perianal nodule
biopsy perianal infammion

Pt did have colonoscopy with bx first.
Then attention after paplating and visualizing the anal canal with no gross disear with the anal canal proper was paid then to the anteroir medline nodule. this had the gross appearance of an inflammatory nodule with some milky, white tissue admixed with salmon-colored material. This was excised in a radially oriented elliptically fashion in the midline after first infiltrating the marcain. this was brought down to the level of the sphincter, but no sphincter injured. The tissue was noted to be quite dense and tough, suggestion chronicrecurrent inflammatory state, suggesting the potential etiology of sentinel tag wchich has become chronically inflamed. then closed.... three separate inflamed areas in the posterior midline, right lateral position, left lateral position were biopsied as well. These had the gross appearance of salmon colored mucosa spreading out from the anal canal in an elliptical fashion into the perianal skin. At the margins of these milky, white, squamous type mucosa was appriciate. in the left lateral position a port of this was biopside with the salmnon colored mucosa.
