What cpt code and place of service would you bill. Patient was scheduled to to have version at local hospital for version. Patient present for version and was found to be in VTX on ultrasound. Extended monitoring was done. NST reactive. Patient sent home to follow with regular scheduled OB appointments

You can bill for the ultrasound if your provider either performed/supervised and/or interpreted it. Otherwise you can bill for an NST (again assuming your provider did the work and interpretation). For a Dx I would suggest Z53.09, procedure not carried out due to contraindication or Z53.29 (other specified reason). Z03.79, Encounter for other suspected maternal and fetal conditions ruled out, is another option. The place of service would be the hospital where the ultrasound and NST was performed (if you can bill for any of this).
