Patient come in for following chemo for Lung cancer (162.9) and bone cancer (198.5).

Decadron 5 mg IV 2:18Pm to 2:37 PM (19 min)
Aloxi 0.25 mg

Alimita (J9305) IV 2.37Pm to 1.53PM (16 min)

Zometa (J3587) IV 2:53 tp 3:30 PM (37 min)

How would this be coded? 96413 (zometa), 96411 x 2 (Aloxi, Decadron, Alimita)? 0r 96413 and 96375?

96413 - Alimta (chemo drug) 16 minutes

96367 x 2 - Zometa and Decadron

96375 - Aloxi (if pushed)

p.s. 96413/96411 are used to document the administrations of chemo drugs (J9xxx); Zometa and Decadron are not chemo drugs.
