For an Office Consultation do you need Request, Reason, Rendering Services, Report and the Referring Provider in the soap note?

Does the provider also have to have any exact wording stated in his note for each of these to make it qualify for a consult?
 
This isn't necessarily a requirement I would say, but I believe it is good practice for the provider to provide a clear reason on why the patient is being referred to them and for what reason they are being seen.
