Wiki Help with correct coding of Cath Report

Okay, procedures listed are:
aorto-ilio bifemoral angiography. sheath shot right femoral. Sheath Shot left femoral. PTA Fem Pop LT. Stent Placement Peripheral LT. Stent Placement Peripheral RT. PTA S and I each add peripheral. Intervention on left superficial femoral artery: percutaneous intervention. Intervention on left superficial femoral artery: stent. Intervention on right superficial femoral artery: stent.
Well, our "in house encounter form" for this particular doctors peripheral procedures is marked for coding like this: 36247-50, 75716-26; 35470, 75962-26; and 37206, 75960-26. I would think that it should be 37205 and 37206, correct? Can anyone also help with particular modifiers needed for this type of coding? I did add -59 to the radiological codes and it passed CCI edit. Thank you. Gail Davis, CPC
 
37206 is an add-on code and cannot be used without 37205 so I agree with you in that regard. But I think you should use 35474 instead of 35470. Also I do not see 75635 which is the aorto-ilio bifemoral angiography.
