I have a physician who gets asked to consult in the hospital. There are times that he has not dictated he was asked to consult. When he sees the patient and starts treatment do I code this as a subsequent hospital visit since it's in the hospital?

I would think that his documentation would dictate how you code this.
If there is no previous record of a hospital visit, why not code an Initial Hospital encounter?
