Hi,

I am confused can anybody tell me what ICD-9 diagnosis do we assign when a report says an established patient came to see his doctor and the doctor removed 5 lesions 4cm. What ICD-9 code would you assign?

Was each lesion 4cm? If not what was the size of each and the location.
how were they removed? Destroyed, shaved, excised?
If the are described as just lesions then 709.8 unless they were excised in which case you hold until the path comes back and code the dx from the path.
You may not assign a 238.x code for neoplasm uncertain behavior, unless that is the dx rendered by path.
