I am in desperate need of help understanding coding units versus how the medications are purchased/packaged. I really thought I understood these until the question came up about how they are packed, our cost versus billing.

Decadron/Dexamethasone is J1100 per 1 mg.
Comes in a 10 mL vial or 100 mg which costs $2.20 per 10 mL vial.
This translates to 1 mL for a 10 mg dose.
My question: is it still 10 units when billed for a 10 mg dose?

Prednisolone is J7510 per 5 mg
Comes in a 5 mL vial or 15 mg.
They normally give 30 mg
Is it 6 units to get 30 mg or 2 because 2 vials used?

Bicillin CR is J0558 per 100,000 units
Comes in a 1,200,000 unit prefilled syringe
Is it 12 units for dose or 2 because 2 syringes used?
This is one that if the dose is less than 1,200,000 units is wasted. From what information I can find in these cases we have to bill for the full 1,200,000 units.

Any help is greatly appreciated!!!

Go by the dosage in the code description and round up if necessary. If a code is 5mg and you give 30mg it's 6 unit. Give 31 and it's 7 units and what's left in the vial is thrown out. I believe there is a way to code wasted medication for tracking purposes but not sure about that part
