:confused: I know when a pt comes in and is seen for a sore throat and the provider sees a skin lesion and decides it needs to be biopsied we append the 25 to the E/M. But what is confusing me is when a pt comes in for a thyroid check and the provider does an ultrasound in the office (not using it for diagnois purpose, and we are not billing out the techinal componet) do we append the 25 to the E/M? And do we append the 26 to the US?

Thank you for any help I can get.
