Not sure how to code this. Don't feel like it fits with 67923 or 67924

I agree with you - this is more involved than just tightening the lid. I'm not an expert on this but it looks more like 67966; stretching a pedicle flap of muscle along the lid margin and anchoring it to the periosteum.
Hopefully an ophthalmic guru will come along soon!
