Wiki Help with spinal fusion surgery coding

I'm hoping someone can help me code this surgery.

L3-4 transforaminal lumbar interbody fusion
Left L4 transpedicular approach
L3-4 diskectomy and anterior interbody fusion
Posterior fusion L3-4

Any help I can get will be very much appreciated!
