I'm looking for some guidance on suspected abuse. A parent is being seen (FAP case) for suspected abuse of their son. The provider is using a code that would be used on a child (T7602XA). Any help would be most appreciated!
That is a pediatric code, and cannot be used on anyone over 17.
Look at the Y07 codes. If biological father, then Y07.11; if bio mother, then Y07.12; if adoptive father Y07.13; if adoptive mother Y07.14. Other family members and stepparents, foster parents, friends, etc., have other codes in the Y07 series.
From what you posted you are coding an examination of a suspected abuser. There is no code for this. The whole innocent until proven thing. You can code suspected abuse of the victim but not suspected perpetrator. You will need to go with any symptoms of behavioral conditions documented such as anger or agitation. In addition Y codes can never be the principle or first listed code. They are status codes., not diagnosis codes.
Sorry, missed the "suspected" part. Look at the Z04 codes.
Z04.42 Encounter for examination and observation following alleged child rape
Z04.6 Encounter for general psychiatric examination, requested by authority
Z04.72 Encounter for examination and observation following alleged child physical abuse
and
Z13.39 Encounter for screening examination for other mental health and behavioral disorders
Also, you mentioned this is a case involving authorities (Family Advocacy Program). Would they not have guidelines specific to their program? Are you billing a program or billing general health insurance?
