Help!!! Help!!! Help!!!
We have billed Medicare for 25 units of 14302 and they deny for too many units. Then we billed it with 1 line with 1 unit and 6 lines with 4 units each (total 25 units). They paid on the 1 unit and 1 line of the 4 units. The rest they denied for duplicate (all have modifier -59 appended).
How do we bill the documented size of the transfer and get paid for the work performed without having to appeal the claim every single time? Talking to Medicare is absolutely no help!
