I have two questions maybe someone can answer them?? Thanks!!

Medicare B (CAHABA) is denying 37250 for primary procedure not identified on the claim? That is the first add on code for IVUS the second add on is 37251 any suggestions? (these are not being billed with coronary procedures but peripheral/vascular procedures) What are the primary procedure codes?

Also Medicare B (CAHABA) is denying 75774 when it is billed multiple time, ex. 75774-26
75774-26-59

Any help would be great..

Hello, the procedure report will be needed to review the coding. Since 37250 and 37251 are both add on codes, the base procedure(s) must be determined.

Additional add on CPT 75774 also requires that the code for the specific initial vessel studied be billed.
