A patient present to the Breast Clinic due to an abnormal mammogram of the right breast. She also has chronic kidney disease and hypertension.

PREOPERATIVE DX: Intraductal Breast Carcinoma of ROQ
POSTOPERATIVE DX: Same

Operative Procedure: Percutaneous Stereotactic Breast Needle Biopsy with clip localization of Right Breast.

I won't give you the answer but I will point you in this direction

Ask yourself:
1) Why are they there? Do I have definitive evidence to support this diagnosis? (This will be your first code)
2) Code all other diagnosis relating to that visit. Be sure to know your combination codes.
3) Code your CPT Codes

This one is simple if you know the coding guidelines. Also you should look up intraductal breast carcinoma if you do not know what it is. Combine that knowledge with the guidelines for the hypertension and the chronic kidney disease and you will have your answer.
