Hello,

The Doctor dictated " TRANSVAGINAL HISTERECTOMY" as diagnosis. To me that is a procedure not a diagnosis. He did not do the procedure, pt had it done by someone else. Do you agree or is there a DX code for that?

Yes, but I don't have my books home today so look under "absence" of reproduction organs, there should be a V code....he should have dictated "status-post transvaginal hysterectomy" indicating absence of uterus, ovaries? cervix? etc...
