patient has had a renal transplant,Z94.0, but doctor still dx codes as N18.6. Is this correct?

It can be correct. Transplant status does not preclude the patient from any dx of renal failure if the eGFR warrants it. I would query the doctor to be sure that the dx is correct and not an old one over looked.
