The doctors at my practice which is orthopedic have asked for a discription of each level of exam and what they have to document to meet that level. Does some one have a cheat sheet that I could use? Or would you know a web site I could find one on. It is so hard to explain the levels and what they need to meet that level. The docs all seem to use the 95 they do not do the bullets we are going over the the ENR system soon but it does not code for them. Thanks so much for some help. Grandma Cora
 
Look at E/M University-- they have great information concerning the levels of history, exam and medical decision making. When I teach my students, I always have them pull up this website and read the informaiton.
