So I think I'm confusing myself...Patient came in because he has had blood in his urine for 2 days. The last episode was yesterday after work and has not noticed any since then. They run a UA and it shows a moderate amount of blood in the urine...Should I code this as gross since the patient noticed the blood or would this be microscopic since the last episode was yesterday but it showed up on the UA? I am trying to give feedback to my providers on how to document better and I'm not sure on this one.
 
I would code gross hematuria since that is what prompted tthe patient to see the physician. Had the patient not seen blood in the urine and it was found on microscopic exam, I would code the microscopic hematuria.
