What are people using for a revision of a shoulder hemiarthroplasty? Hemiarthroplasty done for a fracture. Now the the hemi humerus stem is loose and they they are replacing it. Codes 23473/23474 are revisions for a total shoulder.
 
The work of 23473 completely and accurately describes the surgery, and I would still use that code.
Your alterative would be to either code 23616+22 since it's a revision prosthetic for fracture, or 23999 and compare to 23473, which is risky but technically appropriate.
 
Thank you. Didn't know if modifier 52 should be added because it is was not a total shoulder replacement. Hemi hip guidelines state modifier 52 should be used.

It's a valid question, but it's also the difference between 23473 and 23474, so if you're revising a single component, the work is the exact same, the surgical approach is the exact same, there is no reduction in work whatsoever in 23473 if you're revising a hemi vs a TSA. I would not think a -52 is appropriate here.
