Another practice case... in Carol Buck's book...

Patient having an EGD - the reason is for abnormal upper GI and dysphagia.

The EGD found a hiatal hernia.

The answer says code 43235 (agreeably) and 793.4 and 787.2 (agreeably) however, I also coded the hiatal hernia. The answer doesn't include the hernia.

No rationale is given. Thoughts?!
