I have a hernia size of 3cm x 4cm is this added together o multiplied ?

I think there may be a misunderstanding here. If you are talking about hernia size for CODING, you would neither add nor multiply. It is simply the larger measurement if there is a single hernia.
You never multiply, which would give square cm - useful for debridements/wound care but not relevant to hernia repair.
You would add if there are MULTIPLE hernias.
In your example, a single hernia 3.0cm x 4.0 cm is coded as a 4.0 cm size.
