I think there may be a misunderstanding here. If you are talking about hernia size for CODING, you would neither add nor multiply. It is simply the larger measurement if there is a single hernia.
You never multiply, which would give square cm - useful for debridements/wound care but not relevant to hernia repair.
You would add if there are MULTIPLE hernias.
In your example, a single hernia 3.0cm x 4.0 cm is coded as a 4.0 cm size.
Here is a good article from ACS describing all the 2023 changes, which includes this diagram to clarify:

Just so I understand . If I have a defect thats 4cmx4cm single hernia I would use 49594?
One hernia with a size of 4cmx4cm is sized for coding as 4cm. 49594 could be correct depending on the type of hernia and if it was incarcerated.
