Is anyone familiar with this type of repair?

Incisional hernia repair -recurrent
Does 49568 cover complex wound closure with subcutaneous flaps? I'm not sure I'm reading cpt assistance correctly. It looks like that's included.
Also what about complex repair with myocutaneous Flap? I don't see an edit for 15734 with 49568.

Incisional hernia repair

49568 is an add-on code for the mesh only. So you would add it if mesh was used. The repair codes for recurrent incisional or ventral hernia repair are 49565 (or 49566 if the hernia is incarcerated or strangled). I don't use the wound reconstruction codes. The surgical codes include the closure.

So 49565 (or 49566) + 49568 (if mesh is used).
