If someone goes in for a hernia repair and the documentation also reports a medical history of asthma and allergic rhinitis, do I also code for the asthma and allergic rhinitis or just the hernia repair?

So confused!!!

You would only code those conditions responsible for the encounter and which are to be addressed ie the hernia. The other conditions have no bearing on the hernia repair.
Remember that procedures and diagnoses are linked by medical necessity. If you remember that then you can avoid using irrelevant diagnoses in the future. Only those coding for the hospital (inpatient), rather than the physician, would possibly use these additional codes.
