Can you bill for HH repair 43282 and Lap Sleeve 43775 if the HH is found incidentally? Would it need to be diagnosis prior to the surgery?
 
We've been using 43281 since our surgeon doesn't typically use mesh. We've had some luck; it just depends on the payor...some consider it inclusive, others will pay as long as we use modifier 59.
